- Download the App.
- Enter the Prescription (Rx) number and date of birth or scan the barcode on the medication bottle.
- Enter 732-349-2991 under the pharmacy phone number and select Community Pharmacy and Mobility.
- Select Pickup and send your refill request directly to the pharmacy.
